Sports Physical

Why it's important

Our office offeres sports physicals all year round for any sport. Bring in your school's or sport's paperwork with your child to the visit. If your child has already had their annual Wellness Visit, sports physical paperwork can be filled out for a $25 fee. If your child has not had their annual wellness visit yet, we can combine a wellness visit with a sports physical if your insurance allows. Contact your insurance to find out!
